Guest Assessment Prior to Entry

After having completed a Guest Application Form, Blue Lotus will ask prospective Guests to provide pre-admission documentation comprising:

  • Latest chest X-ray report within prior 6 months.
  • Doctor’s memo / hospital discharge summary indicating his/her medical conditions within prior 6 months.
  • Up-to-date medication prescription list.
  • Vaccination records for last 12 months (patient needs to be fully vaccinated with 2 main doses and at least 1 booster). Exemptions may be allowed on a case-by-case basis to be reviewed during the initial assessment.

Prospective Guests will also be invited to complete what is known as a Barthel Index Assessment Form. The Barthel Index (BI) is a scale that measures a person’s ability to perform basic activities of daily living (ADLs):

  • What it measures: The BI is a 10-item scale that assesses a patient’s independence in areas such as feeding, bathing, dressing, mobility, and more.
  • How it’s scored: The BI is scored from 0 to 100, with higher scores indicating greater independence. A score of 100 indicates total independence.
  • How it’s used: The BI is often used to assess stroke patients and can also be used to assess patients with neuromuscular or musculoskeletal disorders. It can be used to help determine a patient’s likelihood of being able to live independently at home after discharge from the hospital.
  • How it’s administered: The BI can be administered by skilled or unskilled individuals and can even be self-administered by the patient. However, it’s not recommended for patients over 75 years old to self-administer the BI.
  • How long it takes: The BI takes about 5-10 minutes to complete and a copy is set out below for reference.
