Application Procedure for Blue Lotus Care, Bangkok Campus, Explained

After having browsed our website and determined that you would like to apply for accommodation and care services at Blue Lotus Care at Ramkhanhaeng in Bangkok, please access the dropdown menu to access our Guest Application Form which can be completed and submitted HERE.

When considering accommodation for a Guest needing early-stage dementia care, please pay specific attention to the documents which we are requesting from you, as detailed in the lower part of the Application Form. We are not able to proceed with your Application until ALL the requested documents are provided to us. These documents are required prior to acceptance, so that our nursing and medical team can assess admission eligibility and to help design an appropriate care plan according to the Guest’s medical needs. 

For the sake of clarity, the required documents comprise:

  • A chest X-Ray report
  • A completed Barthel Index Form [which you can download with your Application Form]
  • Any recent medical documents (e.g. discharge summary/in-patient summary from hospital, or memo from your General Practitioner)
  • A list of the Guest’s current medications

All medical documents should be dated no earlier than six (6) months from the intended date of admission.

Once we have received all the documents requested, which you either send to us by email at info@bluelotus-wellbeing.net or you can upload complete copies to our website, then we can review your application and make direct contact with you once we have completed our internal review.

Please Note If you wish to secure accommodation at the Blue Lotus Senior Living Campus at Pranburi, you can make your own reservation online, using the booking engine within this website (Book Here). Please note that there is a minimum stay requirement of 14 consecutive nights.

Alternatively, you can call our direct line, as shown on our website and speak to one of our reservation team regarding a booking with us, where we focus primarily on stays of one month, two months and three months, subject to you obtaining the necessary Thai entry visa.
